I understand that Lab Testing Direct is an online direct to consumer lab test purchasing service. I understand that by using this service, I agree to be bound by the terms & conditions set forth by Lab Testing Direct. I agree to use the website according to the instructions.

1. I am over the age of 18. If I am purchasing testing for someone under the age of 18, then I attest that I am either a parent or legal guardian of that person.
2. I attest that the information I submit on the Lab Testing Direct website is truthful and accurate.
3. I understand that no doctor / patient relationship will be formed as a result of the use of the Lab Testing Direct website or the purchase of any lab testing services or products from the website. I also understand that no doctor / patient relationship will be formed as a result of any communication with any Lab Testing Direct staff, employees, or physicians.
4. I understand that the test results obtained from any lab testing procedure is for informational purposes only, and that I intend to take all lab results (positive or negative) to my treating physician for evaluation and interpretation. I understand that the information found on the Lab Testing Direct website is not meant to replace the advice, examination, or care of a qualified physician or health care professional.
5. I understand that the results of a laboratory test do not constitute a diagnosis, and that the information gained from a laboratory test is not meant to diagnose, treat, or cure any disease or condition.
6. As an online service Lab Testing Direct does not provide services, information, or assistance in order to file insurance claims.
7. I understand that lab test results can be inaccurate through no fault of Lab Testing Direct, and understand that lab testing results can have a percentage of false positives or false negatives.
8. I agree to abide by the purchase and refund policies of Lab Testing Direct. Refunds on all tests are only permitted prior to submission of a specimen. Once a specimen has been placed in the mail, or draw from a LabCorp facility then no refund can be given. Refunds on unused LabCorp requisition orders will be provided after one month from receipt of payment. No refunds will be provided after 6 months from receipt of payment on any test.
9. I understand that the services provided by Lab Testing Direct is a service provided at my own request, and not suggested by any Lab Testing Direct staff, employees, or physicians.
10. Indemnification. I agree to indemnify, defend and hold harmless Lab Testing Direct, its officers, directors, employees, agents, licensors, suppliers and any third party information providers to the Service from and against all losses, expenses, damages and costs, including reasonable attorneys' fees, resulting from any violation of this Agreement.
11. Discount coupon codes issued by Lab Testing Direct are issued as a courtesy to our customers, and are available to anyone. Coupons are not redeemable for cash. In addition, the following general service disclaimer regarding individuals in the State of Florida responding to discount codes is as follows:
    THE PATIENT AND ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAS A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR PAYMENT FOR ANY OTHER SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION, OR TREATMENT.
12. Lab Testing Direct offers no assurance of uninterrupted or error free service. Lab Testing Direct shall not be liable for any cost or damage arising either directly or indirectly from any transactions conducted or information conveyed on the website, or via communication with any Lab Testing Direct staff, employees, or physicians. It is solely your responsibility to evaluate the accuracy, completeness and usefulness of all opinions, advice, services, merchandise and other information provided through the service. Lab Testing Direct does not warrant the accuracy or completeness of the information, text, graphics, links or other items contained on the site. Any of the information offered on the site may change at any time without notice. Lab Testing Direct reserves the right to make changes to the terms & conditions at any time. Changes will be effective immediately upon posting of the modified terms & conditions. Permission to use this site is at the discretion of Lab Testing Direct and may be terminated at any time for any reason.
